Keep it short. One to three questions maximum. Longer than that and the customer abandons. Make it conversational. “On a scale of 1 to 5, how was your experience today?” Not “Please rate the following 10 statements.” Ask at the right moment. After a purchase, after an appointment, after a support conversation — when the experience is fresh. Act on the feedback.
